I hereby authorize National Children’s Museum and First Focus on Children, or any individual or entity authorized by the two organizations to take, edit, adapt, post, stream, and/or use photographs, films, or audio or video recordings (herein referred to as recorded materials) of my child or children listed below, and use my child or children’s names and likenesses, in any form and on any media for any purpose, including by way of example news, webcasts, promotional purposes, telecasts, advertising, inclusion on websites, and social media.
I hereby acknowledge National Children’s Museum and First Focus on Children will have all rights, titles, and interests to recorded materials and further acknowledge my authorization and my child or children’s participation is voluntary and will be without payment of fees, royalties, special credits or discounts to National Children’s Museum or other compensation.
I hereby release National Children’s Museum and First Focus on Children and their respective agents, employees, trustees, officers, directors, successors, and assigns from any liability or obligations of any nature whatsoever arising out of or in connection with the taking, recording, editing, publication, distribution or other use of such recorded materials, including but not limited to any liability for defamation, violations of rights of privacy or publicity, infringement of copyright, or any other right respecting the consent provided herein.
I hereby certify that I am the parent or legal guardian of the child or children listed below, who is/are minor(s) under the age of 18 years and that I have full authority to consent and authorize National Children’s Museum and First Focus on Children to take, make, edit, or use, recorded materials of my child or children listed below and use my child or children’s names and likenesses.
